EVENT TICKET INFORMATION FORM
Complete this form to submit information to the Stamp Student Union Ticket Office about your upcoming event.
All questions and correspondence regarding tickets for your event should be sent to
stamp365guest@umd.edu
The Stamp Ticket Office will not manage ticket sales or ticket distribution for events that are not taking place in the Stamp Student Union or an outdoor space not managed by the Stamp Student Union.
This form must be submitted no less than 7 business days
(business days are Monday - Friday)
prior the date when you want tickets to be available for sale/distribution. Failure to submit this form in a timely manner prior to your event, will not be seen as a rationale to exempt your event from using Stamp Ticket Services.
All event fees must be paid in full before tickets will be made available for sale or distribution.
All seating for events will General Admission (no assigned seating or seat selection).
All ticketing information must be finalized
before
tickets are made available for sale or distribution.
The individual submitting this form is the only individual authorized to make any changes.
Once this form is submitted, clients are responsible for payment of the ticket services setup fee.

If you are selling tickets, there are only two (2) options for how you can receive the revenue from your ticket sales.
1.
By Check
: If you select this option, you must submit a completed W-9 form. It can take 4 -6 weeks to receive your revenue check by mail from the State of Maryland. We must have a completed W-9 form before tickets will be made available for sale.
2.
Department Worktag / Student Org Worktag Account
: If your organization is a university department or student organization, you can have your ticket sales revenue deposited into your campus based Worktag account. We must have the account number before tickets will be available for sale.
We are unable to deposit ticket sales revenue into personal or business checking or savings bank accounts.
To receive your ticket sales revenue by check, you must upload a completed W-9 form below.
We must have your completed W-9 form before tickets will be made available for sale.

For UMD student organizations and university department:
To have your ticket sales revenue deposited into a campus Worktag account, please provide the account number below.
If you don't submit a campus Worktag number here, you must upload a completed W-9 form for the previous question.

I have read and understand the following:
I understand that I am the only person from my organization that will be able to make any changes or updates to this ticket information. All communication regarding tickets for this event will be communicated from me and to me.
I understand that no changes to this ticket information can be made once tickets become available for sale or distribution
. The exception would be for a full event cancellation.
I understand that all sales are final, and the Stamp Ticket Office does not offer any refunds, except for a full event cancellation.
There are also no ticket exchange options available.
If there is a full event cancellation (prior to the start of the event), the Stamp Ticket Office will coordinate the refund process for all ticket purchases.
All other email requests for refunds will be forward to me, and it will be the responsibility of my organization to decide whether or not to give a refund, and my organization must coordinate getting a refund to the guest.
I understand, that all event fees must be paid in full prior to tickets being made available for sale or distribution.

To receive a revenue check, I understand that I must submit a W-9 form prior to any tickets going on sale. Once Event Settlement paperwork is submitted following the event, it can take up to 6 weeks to receive your check from the State of Maryland.
UMD Student organizations and university departments have the option to have revenue deposited into their campus Worktag account. That account number must be listed on this form.
The Stamp Ticket Office is unable to deposit ticket revenue into personal checking/savings bank accounts.
I understand that all seating will be general admission (no assigned seating and no seat selection).
I understand that my organization is responsibility of ensuring that all event guests are following facility policies, included any relevant health and safety related requirements.
I understand that my organization will be held financially responsible for any damages that occur in the event space during the event.
